Wolseley Medical Centre is looking for a friendly, reliable, and enthusiastic Health Care Navigator / Admin Assistant Apprentice! This is an exciting opportunity to gain valuable experience in a busy GP practice. As a Health Care Navigator, you will work as part of a multidisciplinary team of clinical and non-clinical staff. You will help coordinate and facilitate effective communication between patients, their carers and the practice, ensuring patients receive an appropriate and timely response to their healthcare needs. The role is predominantly focused on responding to telephone and online requests, providing excellent customer service and helping patients access the most appropriate support. You will need to communicate sensitively and effectively with people from a wide range of backgrounds, including patients whose first language may not be English.

Key Responsibilities

  • Welcoming and assisting patients at reception.
  • Answering telephone calls and responding to patient queries.
  • Responding to online patient requests.
  • Booking, cancelling and amending appointments.
  • Signposting patients to the most appropriate healthcare professional or service.
  • Receiving, recording and passing on accurate messages to colleagues.
  • Making outgoing telephone calls to patients when required.
  • Accurately recording and handling confidential and sensitive information.
  • Processing repeat prescription requests in accordance with practice procedures.
  • Dealing with incoming post and medical records.
  • Filing and maintaining accurate records.
  • Carrying out general administrative and office duties.
  • Working collaboratively with clinical and non-clinical members of the practice team.
  • Maintaining patient confidentiality and professional standards at all times.
