Make a Difference to the Lives of Learners with SEND
We are seeking an organised, proactive and dedicated EHCP & Transitions Coordinator to join our Learner Support team. This is an exciting opportunity to play a pivotal role in supporting students with Special Educational Needs and Disabilities (SEND), ensuring smooth transitions into College and coordinating vital Education, Health and Care Plan (EHCP) processes. Working closely with the Director of Learner Support, Local Authorities, schools, parents/carers and external agencies, you will coordinate EHCP consultations, annual reviews and transition arrangements, ensuring compliance with SEND legislation and the SEND Code of Practice.
About the Role
As an EHCP & Transitions Coordinator, you will lead and coordinate the College's Education, Health and Care Plan (EHCP) processes, ensuring compliance with SEND legislation and the SEND Code of Practice. You will act as a key point of contact for stakeholders and be responsible for managing a high volume of consultations, annual reviews and transition activities. This is a fast-paced role that requires excellent organisation, communication and administrative skills, along with the ability to manage competing priorities and meet statutory deadlines. Key Responsibilities
- Coordinate and manage EHCP annual reviews and consultations, ensuring statutory deadlines are met.
- Act as the main point of contact for Local Authorities, schools, parents/carers and external agencies.
- Support learners with SEND through effective transition planning and arrangements.
- Organise and facilitate annual review and transition meetings.
- Produce high-quality reports, consultation responses and learner documentation.
- Maintain accurate records and support High Needs funding and compliance processes.
Criteria
- Level 2 qualification in Maths and English.
- Experience working with EHCPs, SEND legislation and the SEND Code of Practice.
- Experience managing a high-volume caseload and meeting deadlines.
- Strong communication, report writing and organisational skills.
- Confidence working with Local Authorities, families and external stakeholders.
- Excellent IT skills and experience using databases and Microsoft Office applications.
