HUC is seeking experienced Advanced Nurse/Clinical Practitioners to join our integrated urgent care services across Cambridgeshire, Bedfordshire, Hertfordshire, West Essex, and Somerset. Working in partnership with local healthcare providers, our portfolio includes Out-of-Hours (OOH) GP care, Urgent Treatment Centres (UTCs), and the Acute In Hours Visiting Service (AIHVS), with many sites co-located in hospitals, GP surgeries, and health centres to ensure seamless patient care.
This highly autonomous role is central to delivering safe, effective, and responsive care for a diverse patient population, often managing complex and undifferentiated cases. You will operate within a dynamic multidisciplinary environment alongside GPs, Advanced and Urgent Care Practitioners, Paramedics, and non-clinical staff, ensuring patient safety and service quality. Flexibility to work across various services, locations, and patient presentations is essential. This position offers the opportunity to contribute to service transformation, support equitable access, and help strengthen local healthcare system resilience throughout HUC\βs urgent care portfolio.
Main duties of the job
- Autonomously assess, diagnose, treat, and discharge patients via telephone, in Treatment Centres, UTCs, or during home visits under OOH and AIHVS models.
- Manage a broad spectrum of minor illnesses and injuries, including ENT and ophthalmic cases, using advanced clinical reasoning and evidence-based practice.
- Provide clear health education and tailored self-care advice to support patient understanding and management.
- Rapidly identify and manage acutely unwell patients, initiating urgent or emergency care as needed.
- Recognise safeguarding concerns and act decisively in line with HUC procedures.
- Deliver compassionate support to patients needing palliative or end-of-life care, including medication management.
- Adapt clinical practice to changing patient demand and service pressures, and participate in audits, service evaluations, and quality improvement.
- Prioritise patient care based on clinical risk, acuity, and urgency, ensuring safety and efficiency.
- Exercise autonomy within professional boundaries, comply with NMC/HCPC standards, maintain registration, and uphold HUC\βs governance, revalidation, and appraisal requirements.
- Communicate effectively with patients, families, and multidisciplinary teams, document all clinical interactions accurately, contribute to team meetings, and engage in ongoing professional development.
