Job summary

We are seeking a highly skilled and compassionate Advanced Nurse Practitioner to join our team. This role involves flexible working across four practices, all located within a three-mile radius in the Tameside area. The successful candidate will play a vital role in delivering comprehensive patient care, undertaking clinical assessments, and supporting effective clinical decision-making. We offer one-to-one training, along with weekly tutorials tailored to areas where additional support may be needed. An on-call GP will always be available for guidance. Both full-time and part-time applications will be considered. Candidates must be able to work until 18:30, with flexibility to remain later on occasion in the event of urgent clinical need.

Main duties of the job

  • Conduct thorough patient assessments, including medical histories and physical examinations
  • Provide home visiting service for all four sites – covering a three-mile radius in the Tameside area.
  • Develop and implement individualised care plans in collaboration with patients and multidisciplinary teams
  • Prescribe and administer medications safely in line with established protocols and guidelines
  • Monitor patient progress and adjust treatment plans as required
  • Provide health education and counselling to support patient understanding and self-management
  • Accurately document all clinical interactions within electronic health records
  • Ensure compliance with all legal, ethical, and organisational standards relating to patient care

The successful candidate will play a key role in delivering high-quality, patient-centred care, undertaking comprehensive clinical assessments, and supporting effective clinical decision-making. This is a flexible role, with both full-time and part-time hours considered. We are committed to supporting your professional development, offering one-to-one training and weekly tutorials tailored to individual learning needs. An on-call GP is always available to provide clinical guidance and support. The postholder must be able to work until 18:30, with flexibility to remain later on occasion in response to urgent clinical need.

Person Specification

Qualifications – Essential

  • Registered Nurse with current NMC registration
  • MSc in Advanced Clinical Practice / Advanced Nursing Practice (or equivalent)
  • Independent Prescriber qualification

Experience – Essential

  • Experience of working in a clinical setting with evidence of autonomous practice.
  • Experience of lone working in a home visit setting
  • Ability to make independent decisions

Experience – Desirable

  • Experience in working in General Practice
  • 3–5 years A&E/Urgent Care at Staff Nurse Level or Above
  • 2 years of Independent prescribing
