Housing 21 is seeking an Assistant Care Manager to lead day-to-day care delivery within an Extra Care scheme.

You will ensure residents receive high quality, person-centred care and work with families and carers to tailor plans that respect dignity and independence.

This role focuses on rostering, risk management, and monitoring care quality.
