The Care Home Support Team are enthusiastic committed healthcare professionals that have built excellent working relationships with the care homes in the Borough of Richmond. The team are proud of the service that they have developed and its expansion in improving proactive care for residents, hospital avoidance and for the support and training provided to care home staff.
The Care Home Support Team in Richmond have an exciting new opportunity for you to join our dynamic team as a wellbeing and exercise coach on a 12-month fixed term contract or as a secondment opportunity for a band 4 rehabilitation instructor. You will be travelling to care homes in the Borough of Richmond to provide and support residents exercise programmes, which can be group or 1:1. The post holder will develop and deliver exercise and activity opportunities within Richmond care homes with residents that are at risk of deconditioning. You will work in partnership with care home residents and staff under the direction of the care home support team registered health care professionals.
Main duties of the job
To develop the role, engage with care home residents and staff to promote their health and wellbeing through exercise activities.
To lead on the evaluation of the role.
To develop resources to support care home staff to continue working with residents to reduce the risks of deconditioning and social isolation.
To network across the local health and social care system linking with current schemes finding opportunities to work together to support the sustainability of the project aims and outcomes.
To work autonomously, organising your work, whilst keeping clinically related records.

The Care Home Support Team is part of the Richmond Adults Unplanned and Integrated Care service which aims to reduce unplanned hospital admissions of nursing, residential and LD disability/ MHresidents in LBRUT. The team works in collaboration with a network of health and social care professionals
The service operates 5 days a week, Mon – Fri 8am – 4pm

Qualifications
- Educated to GCSE Standard; A-C including English and Maths
- Practical knowledge acquired through training, short courses and /or experience to BTEC / City and Guilds / NVQ Level 3 in Sports and Exercise

- Educated to degree level in exercise physiology, nutrition or similar health science or equivalent qualification or experience
