Job responsibilities
To provide high quality care based on theneeds of the practice population which includes treatment, screening,preventative care and patient education
To assess, plan, deliver and evaluatecare holistically
To deliver care in accordance with agreedpractice protocols and guidelines
Assist in developing new services andcomplementing and extending the range of services offered to patients
1. RESPONSIBILITIES
Key areas of responsibility willinclude:
Provide treatment and care in conjunction with the GPs or independently as appropriate and according to practice policy and guidelines
Takeresponsibility for own clinics and time management
Review andfiling of own test results and those relating to chronic disease
Advisepatients on general health and minorailments with referral to the GP as necessary
Immunisationsand vaccinations including childhood immunisations
Holidayvaccinations and travel advice
Venepuncture
First Aidfor presenting emergencies
Diagnostictests such as ECG, urinalysis, swabs, blood tests
Responsibilityfor the nursing management of patients with chronic disease/long term conditions including diabetes, CHD, CVA,asthma, COPD, CKD, mental health, learning disabilities, autism and hypertension.
To initiate, develop and implementprotocols in relation to patients with chronic disease
To carry out cervical cytology
Carry put new patient health checks,well man and well woman checks
Actively promote health education andprovide smoking cessation and weight management advice
Provide family planning andcontraception advice
Act as chaperone where required
To promote and run health campaigns asappropriate
Ensure the drugs cabinet and doctorsbags are monitored and kept up to date
To carry out risk assessments inaccordance with COSHH regulations
Keep up to date with practice policiesand procedures, especially in relation to recording systems, administration ofdrugs, child protection, confidentiality, health and safety
Adhere to infection control proceduresand ensure practice policies are up to date
Cleaning of equipment and disposal ofwaste
To be competent in the use of thecomputer ensuring patients details are correctly input in accordance withpractice guidelines
To actively contribute and oversee QOFtargets directly relating to the practice nurse role
To provide vaccination to patients intheir own home where required
Support PCN initiatives when required
3. AUDIT &RESEARCH
Collect routine data and participateand initiate clinical audit
To implement audit and researchfindings to improve clinical practice
To maintain disease registers
4. RELATIONSHIP
Responsible to the Partners forclinical issues and the Practice Manager for administrative issues
To work closely with other members ofthe nursing team, practising good teamwork at all times
Liaise with other team members e.g. adminand reception staff, District Nurses, Health Visitors, Community Matrons,School Nurses etc.
Liaise with secondary care and socialservices to ensure safeguarding, quality and continuity of care
5. ACCOUNTABILITY
Maintain own annual registration withthe Nursing and Midwifery Council and work within the scope of the NMCguidelines
Be personally accountable for ownpractice and promote a professional approach to patients and staff
Acknowledgeany limitation in knowledge and competence and practice within own limits atall times, highlighting any training needs to the Practice Manager
Maintainaccurate records in line with NMC guidelines including data collection,completion of templates and read coding
Maintainown continuous professional development and ensure revalidation and PDP is keptup to date in line with the requirements of the practice and the NMC
Maintainown car insurance ensuring cover is in place for work related activities
6. PRACTICE DEVELOPMENT
Attend and contribute to meetings bothinternally and externally as required
Participatein Practice Development and planning
Attend courses and training asidentified at appraisal and agreed with the Practice Manager
Co-operate/participate with researchwithin the practice
7. PREMISES
Be aware and notify the Practice Manager of any Health and Safety and hazardousconditions
To be familiar with procedures if thefire alarm is raised and assist patients out of the practice without puttingown life at risk
Ensure safety procedures are carriedout in order to maintain a safe environment for employees, patients andvisitors.

Experience
- Experience of working in primary care
Qualifications
- Current NMC registration.
- Trained in chronic disease management.
- Up to date training in childhood, adult and travel immunisation.
- Excellent interpersonal skills.
- Self motivated and resilient.
- Knowledge of infection control procedures.
- Trained in cervical cytology.
- Up to date contraception training including administering depo-provera.
- Able to administer injections such as zoladex and degaralix.
- Knowledge of Emis Web.
- Knowledge of primary care targets.
