The Service is designed to deliver end of life and respite care for patients with specialist palliative care needs that wish to remain in their own home. Care is delivered in the patients' home by Health Care Assistants. The post holder will be responsible for the management of the St Johns Hospice @Home service which provides home care for patients living within our catchment area of Westminster, Kensington, Chelsea, South Brent and Hammersmith and Fulham.
Responsibilities:
- The post holder will be responsible for the overall management of the service.
- You will work with two clinical support nurses and a team of Health Care Assistants.
- You will be supported in your role by the Community Specialist Palliative Care team lead.
