As part of our model of care to ensure 'we have the right people, providing the right care in the right place and at the right time', an opportunity has arisen for a Clinical Nurse Specialist (CNS) or Clinical Paramedic Specialist (CPS) to work as part of our multi-disciplinary team within the Hospice Outreach Service in the community covering West Kent and East Sussex. The Hospice Outreach Team is based in the Pembury site of Hospice in the Weald. The team provides the single point of access for all referrals into all services for Hospice in the Weald. The Hospice Outreach Service provides specialist palliative advice and support to patients and their families. You will be part of a rota of clinicians covering our 7 day a week service offering home-based assessments in addition to telephone support. Our team cares for patients with life-limiting illnesses, focusing on individualised care, improving quality of life and achieving patient-centred goals. We care for people with malignant and non-malignant long-term conditions including heart failure, respiratory disease, frailty and neurological conditions. Our team works collaboratively with external partners to provide seamless care within the patient’s journey; this enhances collaborative working and provides opportunities for shared learning.
Responsibilities
- Provide specialist palliative advice and support to patients and families as part of the Hospice Outreach Service.
- Work as part of a rota to cover a 7-day-a-week service offering home-based assessments and telephone support.
- Operate as an autonomous practitioner within a responsive and resilient team.
- Demonstrate advanced communication skills and be a supportive team member.
Qualifications
- Proven community or in-patient palliative care experience at Band 6 or 7 or equivalent, with experience in related areas such as Paramedic, General Medicine, Neurology, Emergency Medicine, Frailty, Respiratory Care, Cardiology or Oncology.
- Advanced assessment skills and non-medical prescribing, or willingness to undertake as part of professional development.
