Join a committed multidisciplinary team providing compassionate care to people with life-limiting illnesses. This role offers opportunities for personal development and career progression within our community-based Hospice Outreach Service.

We seek clinical staff interested in palliative and end-of-life care, whether experienced nurses or paramedics or those with other clinical backgrounds willing to learn. You will be the first contact for community referrals, providing advice and support to patients and families at home, including care and nursing homes. The role involves a rota covering a 7-day service, including telephone support, clinics, and home assessments.

Our focus is on individualised care, aiming to improve quality of life and patient-centered goals for those with conditions like heart failure, respiratory disease, frailty, neurological conditions, or cancer. We collaborate with other community teams to enhance patient experience and team development.

To succeed in this role, you should be a motivated clinician capable of working independently and as part of a team, with strong communication and presentation skills. This position offers a framework to train and develop into a Clinical Nurse or Paramedic Specialist over two years.
