Horizon Healthcare Homes are currently seeking a suitable candidate to become a Team Leader. The role will be to support individuals with learning and physical disabilities and other complex needs at their 10 bed home in Leeds.
We are looking for people who have a desire to make a real difference in people’s lives; have the energy to contribute to a stimulating and varied environment and are passionate about supporting the people who live at the home to live as independently as possible.
The role is 36 hours per week consisting of 3 x 12 hour shifts from 8am-8pm. Two years experience of working in a residential or other care home setting is essential as is a good working knowledge of supporting people with learning disabilities and/or complex needs. Candidates should have a level 2 health and social care diploma and be willing to work towards the level 3 diploma. All posts are subject to an enhanced DBS check.

In addition to supporting the people who live at the home with all aspects of their daily living, the role of the Team Leader is to carry out tasks designated by the Manager or Deputy Manager, including the leadership and delegation of duties to support workers; promoting good practice at all times and acting as an effective and positive role model to ensure the delivery of outstanding care to the people supported.
Horizon Healthcare Homes Ltd was established in 2007 and currently operate 9 homes across West Yorkshire. We provide high quality support for adults with a learning and or physical disability. All homes within Horizon Healthcare are purpose built and offer great opportunities for the people we support and excellent training packages for our dedicated staff teams.
