The Role

We are looking for an enthusiastic Urgent Care Coordinator, Senior Assessment & Enablement Officer to join us in providing quality services for vulnerable adults. Herefordshire Adults & Communities are developing adult social care services in keeping with the responsibilities under the Care Act 2014.

What will you be doing?

You will be expected to have experience of working in Adult Social Care or a National Health Service organisation and be familiar with the process relating to discharge from hospital. This role is focused on coordinating the Discharge to Assess and the transfer of care process. This role is crucial to ensure that customers are assessed in a timely manner and next steps are identified. The role involves close liaison with health professionals and with colleagues in Adult Social Care Locality Teams and Home First.

About You

The post holder will be central to service developments relating to discharge from hospital to home and will need to progress tasks delegated to them. The post holder will need to be familiar with Excel spreadsheets and compiling reports based on data generated from the process. The post holder will have to manage different demands within tight timescales. It is fast-paced work, and you will need to meet deadlines to ensure cases are progressed. Starting at 8 am on weekdays is an expectation of the role. You will receive regular supervision and work in a supportive environment with other social care staff. Herefordshire is a rural county; some travelling may be required.
