Responsibilities
- Deliver adult social work, including Care Act assessments, person-centered support planning and mental capacity assessments.
- Conduct mental capacity and Section 42 enquiries.
- Apply a strength-based approach to empower individuals and champion independence.
- Collaborate with external agencies and partner organizations.
- Engage in CHC and s117 work to ensure seamless support.
- Participate in regular supervision and support programme.
Qualifications and Experience
- Registered Social Worker with Social Work England and a relevant qualification.
- Current knowledge of legislation, guidance and best practice.
- Experience working with diverse adult populations, including those with physical disabilities, learning disabilities, mental health needs and older adults.
- Track record of effective, person-centered assessment and care management.
- Willingness to pursue further qualifications, such as Best Interest Assessor or Approved Mental Health Professional.
