Social Worker (Adult Social Care)

  • Contract: 12 months Fixed Term
  • Location: Hereford City
  • Hours: 37
  • Salary: £36,363 to £40,777 + £3,000 welcome payment

Responsibilities

  • Deliver adult social work, including Care Act assessments, person-centered support planning and mental capacity assessments.
  • Conduct mental capacity and Section 42 enquiries.
  • Apply a strength-based approach to empower individuals and champion independence.
  • Collaborate with external agencies and partner organizations.
  • Engage in CHC and s117 work to ensure seamless support.
  • Participate in regular supervision and support programme.

Qualifications and Experience

  • Registered Social Worker with Social Work England and a relevant qualification.
  • Current knowledge of legislation, guidance and best practice.
  • Experience working with diverse adult populations, including those with physical disabilities, learning disabilities, mental health needs and older adults.
  • Track record of effective, person-centered assessment and care management.
  • Willingness to pursue further qualifications, such as Best Interest Assessor or Approved Mental Health Professional.
