Responsibilities

  • In this vital role, you will work directly with children and young people who have a range of additional needs, along with their families, to ensure they receive the right support at the right time.
  • You will complete holistic assessments, plan and coordinate services, and work collaboratively with families and partner agencies to promote positive outcomes and independence.

Qualifications

  • Passionate about supporting children and young people with disabilities and their families to achieve the best possible outcomes.
  • Skilled in assessment, planning, and coordinating multi-agency support, ensuring that each child’s individual needs are fully understood and met.
  • Committed to working collaboratively, inclusively, and creatively with children, families, and professionals from health, education, and social care.
  • Resilient, reflective, and confident in managing complex situations and making child-centred decisions.
  • Able to work effectively within a supportive and specialist team environment, contributing to a culture of learning and continuous improvement.
  • Registered with Social Work England and hold a recognised Social Work qualification (Degree/DipSW/CQSW).
