About The Role

Assessment and Review Officer

We’re looking for a caring, proactive and skilled Assessment and Review Officerto join our Home First team.

You’ll play a key role in helping adults regain confidence and independence after illness, injury or a period of reduced ability — supporting them to achieve their personal goals and live fulfilling lives in their own homes.

About the Role

As part of our Reablement Service, you’ll

  • Complete timely assessments, reviews and support plans that reflect each person’s needs, goals and strengths.
  • Coach and empower people to take an active role in their own recovery, using a reablement approach that builds confidence and independence.
  • Assess and manage risk, ensuring safe, person-centred care.
  • Work closely with colleagues, families, carers and partner agencies to deliver consistent and effective support.
  • Support the deployment of care staff and contribute to the day-to-day delivery of a high-quality, CQC-compliant service.
  • Teach and demonstrate the use of mobility aids and assistive technology.
  • Maintain accurate, up-to-date electronic records and documentation.

About You

You’ll be an experienced care professional with

  • Experience of assessing adult care needs, risk management and care planning.
  • A background in social care, health care or community settings.
  • Excellent communication, problem-solving and decision-making skills.
  • The ability to work flexibly — independently and as part of a team — to meet service needs 7 days a week.
  • A full driving licence and access to your own vehicle.

You’ll also be self-motivated, compassionate, and confident in supporting vulnerable adults while maintaining the highest standards of safeguarding and professionalism.

Qualifications

  • Diploma/NVQ Level 3 in Health or Social Care (or willingness to work towards).
  • Competency certificates in care, manual handling, and simple aids/adaptations (or willingness to obtain).
