The role
Working within the Living Well commissioning team you will form part of an Accommodation Support Team with a specialist Housing Officer. You will work closely with Adult Social Care and Commissioners to find tailored accommodation and support solutions to enable people with care needs to find a home and to manage the move into their home. Liaison with housing and care providers will be key to ensure smooth transitions out of existing into new accommodation.
