About The Role
Our Children with Disabilities (CWD) Service is a dedicated, supportive, and skilled team made up of a Team Manager, two Managing Practitioners, a Senior Practitioner, Social Workers, and a group of committed Family Support Workers.
As part of our Children and Young People with Disabilities team, you’ll bring your experience, compassion, and creativity to a service that is both fast‑paced and deeply rewarding. You’ll be working alongside social workers and family support workers to support children, young people, and their families across our 0–25 service.
What You’ll Be Doing
- Holding responsibility for a caseload of children and young people.
- Reviewing and developing family assessment and support plans to ensure they reflect each child’s needs, strengths, and aspirations.
- Overseeing and reviewing short breaks packages to support children to access their community and the wider world.
- Working collaboratively with families and social workers to develop meaningful plans that help families achieve positive outcomes.
- Provide skilled direct work to children and families as part of their support plan.
About You
- Qualified to Level 4 or above in health, education, or social care.
- Experienced, at least 12 months working with children and families.
- Confident working in complex environments with a flexible and solution‑focused approach.
- Passionate about improving the lives of children, young people, and families in Herefordshire.
- Confident in understanding the needs of individuals with disabilities, be experienced or willing to train in using alternative communication methods and feel confident in working as part of a wide multi‑agency team to ensure children’s needs are understood and met.
- Passionate about making a positive difference for children and young people with disabilities and their families through high‑quality, person‑centred support and assessment.
- Skilled in analysis, planning, and decision‑making to ensure each child’s individual needs, abilities, and circumstances are clearly understood and effectively met.
- Committed to working collaboratively, inclusively, and creatively with children, families, carers, and partner agencies to promote independence, inclusion, and well‑being.
- Resilient, reflective, and confident in managing complex and sensitive situations, making sound professional judgments in the best interests of each child.
- Able to thrive in a supportive, fast‑paced team environment, contributing to a culture of empathy, learning, and continuous improvement in outcomes for children with disabilities.
