Hoop Social Work is recruiting for an experienced Qualified Social Worker to join the Hospital Team based at St Woolos Hospital in Newport. This is a fantastic opportunity to work within a supportive, fast-paced multidisciplinary team, with a hybrid working arrangement combining time on-site and working from home.

As a Hospital Social Worker, you will play a key role in supporting timely, safe hospital discharges by completing strength-based assessments and coordinating appropriate care and support plans. You will work closely with health professionals, patients and families to achieve positive outcomes while promoting independence.

  • Complete assessments in accordance with the Social Services and Well-being (Wales) Act.
  • Undertake hospital discharge planning in a busy acute setting.
  • Identify potential Continuing Healthcare (CHC) triggers and contribute to the assessment process.
  • Work collaboratively within a multidisciplinary team to support effective discharge planning.
  • Complete person-centred, outcome-focused assessments and care planning.
  • Liaise with a range of statutory and voluntary agencies to ensure seamless transitions from hospital.

Requirements:

  • A recognised Social Work qualification.
  • Registration with Social Care Wales or Social Work England but we are willing to register with SCW ahead of starting.
  • Experience working within adult services, ideally within a hospital or discharge setting.
  • Knowledge of the Social Services and Well-being (Wales) Act.
  • An Enhanced DBS (registered on the Update Service or willingness to apply).
