Hoop Social Work is recruiting for an experienced Qualified Social Worker to join the Hospital Team based at St Woolos Hospital in Newport. This is a fantastic opportunity to work within a supportive, fast-paced multidisciplinary team, with a hybrid working arrangement combining time on-site and working from home.
As a Hospital Social Worker, you will play a key role in supporting timely, safe hospital discharges by completing strength-based assessments and coordinating appropriate care and support plans. You will work closely with health professionals, patients and families to achieve positive outcomes while promoting independence.
- Complete assessments in accordance with the Social Services and Well-being (Wales) Act.
- Undertake hospital discharge planning in a busy acute setting.
- Identify potential Continuing Healthcare (CHC) triggers and contribute to the assessment process.
- Work collaboratively within a multidisciplinary team to support effective discharge planning.
- Complete person-centred, outcome-focused assessments and care planning.
- Liaise with a range of statutory and voluntary agencies to ensure seamless transitions from hospital.
Requirements:
- A recognised Social Work qualification.
- Registration with Social Care Wales or Social Work England but willing to register with SCW ahead of starting.
- Experience working within adult services, ideally within a hospital or discharge setting.
- Knowledge of the Social Services and Well-being (Wales) Act.
- An Enhanced DBS (registered on the Update Service or willingness to apply).

- £37.34 per hour Umbrella.
- Hybrid working - a mixture of on-site working at St Woolos Hospital and working from home.
- Initial 3 month contract with the possibility of extension.
