Hoop Social Work are recruiting for a qualified Social Worker to join the Monnow Vale Social Work Team in Monmouthshire on a full-time locum basis. This role is covering a vacant post and is initially expected to last for 3 months, supporting the Raglan, Usk and Monmouth localities.
Based from Monnow Vale, the successful candidate will undertake a varied community-based caseload including:
- Adult Social Care assessments and reviews
- Reablement and independence-focused interventions
- Duty work and safeguarding responsibilities
- Community-based case management
- Residential and care home-related work
- Multi-agency working to support adults with care and support needs
The role is office-based between community visits and requires a proactive, organised social worker who is confident managing a busy caseload.
To apply for this role, you will need to hold:
- A minimum of 6 months' experience working in the UK as a registered Social Worker
- A relevant Social Work qualification
- Registration with Social Care Wales or Social Work England
- Enhanced DBS registered on the Update Service, or willingness to obtain one
- 3 years of referencing
- A full UK driving licence and access to a vehicle
