This is a fast-paced team environment, so it’s important that applicants are confident hitting the ground running. The role will involve:

  • Managing a caseload of approximately 10-12 Home care cases
  • MCA work
  • Safeguarding
  • Hospital discharges
  • Community assessments

Working pattern: Initially this will be fully onsite whilst getting up to speed, with the option to work 2 days from home / 3 days onsite once settled into the role.
