Social Worker / Senior Practitioner – Young Adults Transitions Service
Bristol City Council | 5-month contract
Hoop Social Work is recruiting an experienced Social Worker / Senior Practitioner to join Bristol City Council’s Young Adults Transitions Service, supporting young people with additional needs as they prepare for adulthood and work towards greater independence.
The citywide service works with young people aged 14–25, helping them maximise their independence through assessments, occupational therapy support, care planning and access to appropriate services and resources.
This is an excellent opportunity for an experienced practitioner who enjoys working with young people and their families, supporting colleagues and contributing to the development of a specialist transitions service.
The role
As a Senior Practitioner, you will play an important role within the service’s leadership team, supporting Team Managers to develop the transitions service and providing day-to-day consultation, guidance and direction to practitioners.
You will help ensure young people receive appropriate, person-centred support as they move into adulthood, promoting independence and making the best use of commissioned services, charitable organisations, technology and equipment.
You may hold a small caseload of complex and challenging cases, leading by example and modelling best practice.
The role also includes supervising staff across the multidisciplinary team, including Social Workers, Occupational Therapists, Social Care Practitioners and Occupational Therapy Aides.
You will need a calm, solution-focused approach, with the ability to manage competing priorities and provide confident professional guidance.
Key responsibilities
- Complete Care Act assessments for young people with additional needs transitioning into adulthood.
- Support young people to maximise their independence through appropriate assessments, care planning and occupational therapy support.
- Develop support plans and contribute to the commissioning of services to meet assessed needs.
- Identify alternative sources of support, including charitable organisations, technology and equipment to promote independent living.
- Provide day-to-day consultation, professional guidance and supervision to staff across the multidisciplinary team.
- Support Team Managers with the ongoing development and improvement of the transitions service.
- Manage a small caseload of complex cases where required, modelling high standards of practice.
- Apply relevant legislation and procedures, including the Care Act 2014 and Mental Capacity Act.
- Contribute to adult safeguarding practice and undertake visits across the city and, where required, out of county.

Contract: 5 months
Location: Bristol City Hall
Office attendance: Minimum of 2 days per week, likely to be 3 days due to duty manager responsibilities.
