The role covers a 9-month initial contract with 37 hours per week, and potential for a 4-day week. Office-based with some travel across the Taf area, driving licence and car required.
You will assess, plan and review care, co‑produce treatment plans, complete MCA assessments, maintain records, and work with partner agencies to promote independence and wellbeing.
