Qualified Social Worker β Community Mental Health Team (CMHT), Neath Port Talbot Council.
Hybrid Working | 4 Days Office / 1 Day Home (subject to service needs). Β£38.00 per hour (Umbrella). 37 Hours per Week | Full-Time | Maternity Cover.
Hoop Social Work is partnering with Neath Port Talbot Council to recruit an experienced Qualified Social Worker to join the Community Mental Health Team (CMHT) on a full-time maternity cover contract.
Role
As a CMHT Social Worker, you will manage a varied caseload of adults with complex needs, undertaking strengths-based assessments, care planning and reviews while working closely with health colleagues and partner agencies to achieve positive outcomes.
Responsibilities
- Completing Care and Support Needs Assessments and developing person-centred care plans.
- Managing a complex caseload in line with local priorities and Positive Risk Taking principles.
- Working collaboratively with health professionals, families, carers and partner agencies.
- Undertaking Mental Capacity Assessments, Best Interest Assessments and Continuing Healthcare (CHC) Assessments.
- Identifying community resources and coordinating support to maximise independence.
- Carrying out safeguarding enquiries and initiating appropriate action where adults are at risk.
- Chairing reviews, professional meetings and case conferences where appropriate.
- Maintaining accurate and timely electronic case records and documentation.
- Monitoring and reviewing care plans to ensure identified outcomes continue to be met.
- Supporting colleagues with complex casework and contributing to service development.
- Participating in multi-agency working and promoting best practice across services.
- Undertaking Approved Mental Health Professional (AMHP) duties where qualified and required by the service.
Qualifications and Requirements
- A recognised Social Work qualification (Degree, Dip SW, CQSW or equivalent).
- Current registration with Social Care Wales or Social Work England.
- A minimum of 6 months' UK experience working as a registered Social Worker.
- Previous experience within Adult Services and Community Mental Health.
- Experience of complex assessments, safeguarding and multi-agency working.
- An Enhanced DBS registered on the Update Service, or willingness to obtain one.
- The ability to provide 3 years of references.
- A full UK driving licence with access to a vehicle.
