Hoop Social Work seeks a Qualified Social Worker to join the Community Mental Health Team (CMHT) in Neath Port Talbot on a full-time maternity cover contract. The post offers hybrid working: 4 days in the office and 1 day at home, at a rate of £38.00 per hour (Umbrella) for 37 hours per week.
You will manage a varied caseload of adults with complex needs, conduct strengths-based assessments, care planning and reviews, and work closely with health colleagues and partner agencies to achieve positive outcomes.
