An exciting opportunity has become available for an experienced Qualified Social Worker to join Cardiff’s Adults with Learning Disabilities Team. This role is available due to an existing agency worker relocating, offering a smooth handover into a well-established caseload.
This is a great opportunity for someone who is confident in Learning Disabilities work and is looking to step into a supportive team where strengths-based practice is at the heart of everything they do.
About the role
You’ll work with adults with learning disabilities and their families, focusing on promoting independence, wellbeing, and meaningful outcomes. The team is committed to person-centred approaches that empower individuals to live fulfilled and purposeful lives.
Your responsibilities will include:
- Completing assessments and reviews
- Developing and managing care and support plans
- Delivering person-centred and strengths-based interventions
- Working collaboratively with families, carers, and partner agencies
- Managing your own caseload and meeting performance targets
The role is currently hybrid/home-based, with office presence expected in Cardiff (Market Road, Canton). Typically for the first month you will be on-site and then 2 or 3 days per week thereafter.
What we’re looking for:
- Qualified Social Worker with Social Care Wales or Social Work England registration
- Experience within Learning Disabilities services (essential)
- Strong background in assessment, care planning, and care management
- Confident IT skills and ability to manage competing demands
- A positive, proactive approach and strong professional values
Requirements:
- Minimum 6 months UK Social Work experience
- Relevant Social Work qualification
- Enhanced DBS (on update service or willing to obtain)
- Full driving licence and access to a vehicle
- Ability to provide 3 years’ referencing
