Bridgend County Borough Council (Pyle) is seeking experienced Adults Social Workers to join the Integrated Network Teams on an initial 3-month contract.

Preferred Hybrid Working – 2 Days Home / 3 Days On Site.

The Role:

  • Working with adults aged 18+ who may be experiencing frailty, long-term health conditions, physical disabilities, learning disabilities, and mental health or emotional wellbeing needs.
  • Managing a varied community-based caseload.
  • Completing strengths-based assessments and care planning.
  • Care coordination and safeguarding responsibilities.
  • Supporting adults to maximise independence and wellbeing.
  • Multi-agency partnership working.
  • Undertaking regular home visits across the locality.

About the Team:

You will join a well-established Integrated Network Team made up of professionals. The team works closely alongside GPs, Primary Care Networks, community health services and Third Sector organisations to deliver person-centred support to local residents.

What We're Looking For:

  • Qualified Social Worker with relevant post-qualifying experience.
  • Registration with Social Care Wales (or Social Work England).
  • Experience completing assessments, care planning and care coordination.
  • Confidence working within a multi-disciplinary team environment.
  • Strong communication and relationship-building skills.
  • Full UK driving licence and access to a vehicle.

The Offer:

  • Up to £35.32 per hour (dependent on experience).
  • Initial 3-month contract.
  • Full-time preferred, with part-time applications considered.
  • Hybrid working arrangement (2 days home working / 3 days office and community-based).

To Apply for This Role, You Will Need:

  • A minimum of 6 months' experience working in the UK as a registered Social Worker.
  • A relevant Social Work qualification.
  • Registration with Social Care Wales or Social Work England.
  • Enhanced DBS registered on the Update Service, or willingness to obtain one.
  • 3 years of satisfactory references.
  • Full UK driving licence and access to a vehicle.
