We are currently supporting Bath & North East Somerset with the recruitment of an experienced Social Worker to join their Hospital Social Care Team on a short‑term basis. This role will focus on supporting individuals being discharged under Pathway 2 and Pathway 3 of the Discharge to Assess process, helping to ensure safe and timely transitions from hospital settings into the community.

This is a hybrid opportunity; however, candidates must live locally to the BANES area due to the requirement to attend the office twice weekly and respond to urgent visits when needed.

We are looking for Social Workers who have previous experience within hospital discharge/community adult social care settings and who are confident managing fast‑paced statutory work.
