Honiton Surgery are looking for an experienced diabetes nurse to join our practice team.
We are looking for someone for 19hrs a week, to lead on the management of our diabetes patients. This would suit someone with experience of working with diabetes patients in a hospital, community or primary care setting.
Whilst Diabetes is the main focus of this role, additional hours up to fulltime, may be available for a suitably qualified individual.
Main duties of the job Working with the nursing team, as a member of the practice multidisciplinary team, the post holder will ensure practice diabetes services are delivered effectively to the our patients.
Your main responsibilities will include:
Delivering nurse led clinics to assess, diagnose, plan, implement and evaluate treatment/interventions for patients for patients presenting with diabetes, pre‑diabetes or undifferentiated diagnosis.
In collaboration with other members of the practice team, lead the planning and delivery of diabetes services, promoting integrated and seamless pathways of care.
Initiate Insulin and GLP1s where indicated.
Provide support and mentorship to other colleagues within the practice.
Contribute to practice targets both local and national, e.g. QOF.
Refer patients to other service/agencies as appropriate.
Promote health and well‑being through the use of health promotion screening.
About us We are a very busy, well‑established GP practice of 17,749 patients and are located in the market town of Honiton in East Devon. We have 5 GP Partners, 13 Salaried GPs, 1 Advanced Nurse Practitioners, 1 Lead Practice Nurse, plus all the associated teams that incorporate our workforce. We have active participation in PCN development with two other local practices and have a good rating with CQC.

Job responsibilities The following are the core responsibilities of the Diabetes Specialist Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
a. To operate nurse led clinics to assess, diagnose, plan, implement and evaluate treatment/interventions for patients for patients presenting with diabetes, pre‑diabetes or undifferentiated diagnosis.
b. In collaboration with other members of the practice team, lead the planning and delivery of diabetes services, promoting integrated and seamless pathways of care.
c. Initiate Insulin and GLP1s where indicated.
d. Provide support and mentorship to other colleagues within the practice.
e. Contribute to practice targets both local and national, e.g. QOF.
f. Refer patients to other service/agencies as appropriate.
g. Promote health and well‑being through the use of health promotion screening.
h. Undertake home visits for patients with diabetes as required.
i. To maintain accurate clinical records in conjunction with extant legislation.
j. To ensure clinical codes are used effectively by all members of the nursing team.
k. To ensure the appropriate use of chronic disease registers.
l. Work to support compliance and adherence to prescribed treatments by detecting and managing side effects and/or adverse reactions.
m. To request pathology services and process pathology results as required.
n. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
o. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).
In addition to the primary responsibilities, the Diabetes Specialist Nurse may be requested to:
a. Participate in local initiatives to enhance service delivery and patient care.
b. Support and participate in shared learning within the practice in order to improve patient care.
c. Continually review clinical practices, responding to national policies and initiatives where appropriate.
d. Participate in the review of significant and near‑miss events applying a structured approach, i.e., root cause analysis (RCA).
e. Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation.
Experience 2 years experience of diabetes management in primary care or a community or hospital setting.
Experience of Chronic Disease Management
Experience of working autonomously
Experience working as a practice or community nurse
Experience of managing diabetes
Experience of managing elderly and housebound patients
Experience of quality initiatives
Knowledge and Skills Wound care/removal of sutures and staples
ECGs
Venepuncture
New patient medicals
Requesting pathology tests and processing the results, advising patients accordingly
Understanding the importance of evidence-based practice
Understand the requirement for PGDs and associated policy
Ability to record accurate clinical notes
Ability to work within own scope of practice and understanding when to refer to GPs
Chaperone procedure
Immunisations (routine, childhood and travel)
Chronic disease management in Diabetes
Excellent communication skills (written and oral)
Effective time management (planning and organising)
Ability to work as a team member and autonomously
Good interpersonal skills
Problem solving and analytical skills
Ability to follow clinical policy and procedure
Strong IT skills and competent in clinical IT systems
Broad knowledge of clinical governance
Knowledge of public health issues in the local area
Awareness of issues within the wider health arena
Knowledge of health promotion strategies
Experience with audit and able to lead audit programmes
Experience with clinical risk management

Qualifications Registered Nurse
Postgraduate Diploma or Master\’s level Diabetes Qualification
Nurse Prescriber
