Duty Practitioner (Males and Females Scheme) Stoke-on-Trent £25,451.50 As a Duty Practitioner, you'll support people living in our male or female supported accommodation schemes who have previously experienced homelessness. Acting as a key point of contact, you'll help customers settle into their homes, develop the skills and confidence needed to live independently, sustain their tenancy and access the support they need to achieve their goals. What you’ll do: * Be the friendly face of support, helping people access the advice, services, and practical assistance they need. * Build positive relationships and work alongside customers to overcome challenges and achieve their goals. * Connect with partner agencies to make sure customers receive the right support at the right time. * Manage a varied caseload, creating tailored support plans and keeping progress on track. * Empower people to build confidence, increase independence, and create positive change in their lives. Who you are: * Educated to GCSE level with a good level of Maths and English * Experienced in providing front-line support within busy service environments, confidently engaging with the public and managing complex behaviours from people facing multiple disadvantages (desirable) * Passionate about making a difference and supporting people to achieve positive outcomes. * Knowledge of basic health and safety * Experience of multi-agency working and working in supported housing. * A great communicator who can build trusting relationships with customers, colleagues, and partner agencies. * Calm, organised, and confident managing a varied workload in a fast-paced environment. * Empathetic and person-centred, with the ability to understand individual needs and provide tailored support. * Motivated, resilient, and committed to helping people build independence and improve their wellbeing.
