HomeLink Healthcare is looking for an experienced Registered Nurse to join our Kettering team as a Discharge Liaison Nurse, supporting the safe and timely transition of patients from hospital into our Hospital at Home and community services. This is an ideal opportunity for an experienced nurse who enjoys combining clinical assessment with patient coordination and multidisciplinary working.

As a Discharge Liaison Nurse, you will work closely with hospital teams, patients, families and HomeLink Healthcare colleagues to assess patients for our services and coordinate safe transfers of care into the community.

About the Role

As a Discharge Liaison Nurse, you will play an important role in supporting patient flow and ensuring patients can move safely and effectively from hospital into appropriate community-based care. You will:

  • Work closely with hospital teams to facilitate the safe and timely transfer of patients from hospital into HomeLink Healthcare's community services
  • Undertake holistic clinical assessments to determine patients' suitability for community-based care
  • Assess patients for pathways including virtual ward and early supported discharge services
  • Review patients' medical history, current clinical needs and ongoing care requirements
  • Identify potential barriers to discharge and work collaboratively with the wider multidisciplinary team to resolve them
  • Coordinate discharge and transfer plans, ensuring appropriate arrangements are in place before patients move into the community
  • Liaise closely with Trust multidisciplinary teams and HomeLink Healthcare clinical teams
  • Complete and document clinical handovers accurately and in a timely manner
  • Provide advice, information and support to ward teams and other healthcare professionals
  • Support patients and families to understand their care options and make informed decisions about their ongoing care
  • Undertake clinical duties as an autonomous practitioner within your scope of practice
  • Support the Clinical Lead and wider team with the safe and effective delivery of the service
