HomeLink Healthcare is looking for an experienced Community Registered Nurse to join our Hospital at Home service, delivering high-quality care to NHS patients across the East of England, within the Great Yarmouth area.
This role is ideal for Community Nurses, District Nurses, IV Therapy Nurses or experienced Registered Nurses looking for flexible bank work while continuing to provide meaningful patient care.
As a Community Bank Nurse, you will deliver safe, effective clinical care to patients in their own homes, helping support earlier discharge from hospital, prevent readmission and enable patients to recover safely within the community.
Patients on this pathway will be discharged from James Paget University Hospital, so you will need to live within, or be able to comfortably travel across, a 15-mile radius of Great Yarmouth (NR31 6LA).

About the Role
• Deliver safe, high-quality nursing care in patients’ homes
• Undertake holistic patient assessments, care planning and evaluation
• Conduct 30–60 minute patient visits, depending on individual clinical needs
• Deliver IV therapy as a primary part of the patient pathway
• Undertake blood tests, patient monitoring, welfare visits and packages of care where required
• Monitor patient conditions and escalate concerns when required
• Maintain accurate clinical documentation using digital systems
• Work collaboratively with GPs, consultants and multidisciplinary teams
• Support patients to recover safely and remain independent at home
About You
You will be a Registered Nurse with a valid NMC registration, current revalidation and a minimum of three years' post-registration experience in community, acute or equivalent nursing settings.
You will be confident working autonomously within the community, with strong clinical judgement and excellent communication skills.
Compassionate and collaborative, you will build effective relationships with patients, carers and colleagues while delivering high standards of patient-centred care.
As this is a mobile community-based role, you will also need a full UK driving licence, access to a vehicle and appropriate insurance.
Essential Clinical Skills
• Competency in IV therapy, including the administration of intravenous medications
• Experience of community nursing, lone working or delivering care autonomously
• Ability to undertake patient monitoring and recognise and escalate deterioration
• Ability to work independently and safely within a community setting
