Discharge Liaison Nurse Location: Kettering, NN16 8UZ On-site and community-based role travel required Salary: from £39,000 Contract: Permanent, full-time Monday to Friday HomeLink Healthcare is looking for an experienced Registered Nurse to join our Kettering team as a Discharge Liaison Nurse, supporting the safe and timely transition of patients from hospital into our Hospital at Home and community services. This is an ideal opportunity for an experienced nurse who enjoys combining clinical assessment with patient coordination and multidisciplinary working. As a Discharge Liaison Nurse, you will work closely with hospital teams, patients, families and HomeLink Healthcare colleagues to assess patients for our services and coordinate safe transfers of care into the community. You will: Work closely with hospital teams to facilitate the safe and timely transfer of patients from hospital into HomeLink Healthcare's community services Undertake holistic clinical assessments to determine patients' suitability for community-based care Assess patients for pathways including virtual ward and early supported discharge services Review patients' medical history, current clinical needs and ongoing care requirements Identify potential barriers to discharge and work collaboratively with the wider multidisciplinary team to resolve them Coordinate discharge and transfer plans, ensuring appropriate arrangements are in place before patients move into the community Liaise closely with Trust multidisciplinary teams and HomeLink Healthcare clinical teams Complete and document clinical handovers accurately and in a timely manner Provide advice, information and support to ward teams and other healthcare professionals Support patients and families to understand their care options and make informed decisions about their ongoing care Undertake clinical duties as an autonomous practitioner within your scope of practice Support the Clinical Lead and wider team with the safe and effective delivery of the service Deputise for the Clinical Lead where required Participate in the on-call rota where required to support service continuity Contract: Permanent, Monday to Friday About You You will be an experienced Registered Nurse who is confident undertaking clinical assessments and making informed decisions about patients' ongoing care. You'll be comfortable working autonomously while also collaborating closely with hospital teams, multidisciplinary colleagues, patients and families. Compassionate and organised, you'll be able to manage competing priorities while maintaining a strong focus on patient safety, communication and continuity of care. You will also need: Active NMC registration A minimum of three years' post-registration nursing experience Recent experience caring for adults with a broad range of clinical conditions Strong clinical assessment and decision-making skills Excellent communication and multidisciplinary working skills Confidence working autonomously across hospital and community settings Understanding of safe discharge planning and transfer of care Knowledge of the Care Act 2014 and relevant legislation relating to hospital discharge Full UK driving licence and access to your own vehicle Right to work in the UK (sponsorship is not available) Desirable: Previous experience in discharge planning, patient flow, community services or Hospital at Home Experience of virtual wards or early supported discharge pathways Previous management, leadership or staff supervision experience About HomeLink Healthcare HomeLink Healthcare is a clinician-led Hospital at Home service provider working in partnership with NHS organisations, private healthcare providers, consultants, community services and GPs.
