Overall purpose of the job

To deliver varied wellbeing programmes designed to promote motivation and the ability to live a full, healthy lifestyle and engage in meaningful activities. This will involve partnership working with agencies, linking residents to services for specific support. The role will involve building trust and assessing residents' needs to ascertain appropriate support.

You’ll work as part of a holistic team to research, plan, organise and facilitate a holistic wellbeing programme to be delivered to enable rough sleepers to improve their mental health, physical and emotional wellbeing. Collaborating with local agencies will be a vital component of the role to deliver maximum impact in helping residents sustain their accommodation and achieve a better quality of life. To begin to build a therapeutic environment centred around the resident’s input with a focus on community.

Working hours: 37.5 hours weekly Monday to Sunday between the hours of 8am and 9pm including weekends and bank holidays. Actual days, start/finish times may vary in accordance with the service rota.

Key responsibilities:

  • Link residents into the community using a holistic approach engaging them to improve their health and wellbeing.
  • Striving to make a positive impact on people’s lives and tailor support to enable people to fulfil their best potential.
  • Build and maintain professional relationships with external partners and stakeholders.
  • Identify areas of support within the community and mental health services that supports the residents’ goals.
  • Deliver and record outcomes that the funding stipulates.

Main duties & responsibilities:

  • Create the planning and delivery of a variety of social, leisure and recreational activities and opportunities based on identified need.
  • Support residents to access a wide range of activities that increase self-confidence, improve health & wellbeing and develop communication and social skills.
  • Develop positive and trusting relationships with residents to support them to fully engage in activities and to remain as independent as possible.
  • Provide outcome focused, person‑centred support, empowering people to become active members of their community.
  • Support individuals to increase their life skills involving a variety of housing support needs, promoting communication essential for decision making and choice.
  • Promote confidence and self‑esteem, helping residents who feel socially excluded to feel included in their community.
  • Promote positive mental and physical wellbeing through preventative and early intervention approaches, equipping people with skills to live independently.
  • Provide practical support with food preparation, budgeting and other daily living skills.
  • Promote participation and inclusion through innovative, creative and personalised interventions.
  • Actively engage with residents to deliver a person‑centred service that is meaningful and valued.
  • Motivate clients to engage with and take ownership of their health and wellbeing.
  • Line manage volunteers.
  • Attend to all administrative duties related to the post.

Person Specification

Essential:

  • Experience of supporting adults using a person‑centred approach.
  • Experience of facilitating and delivering a range of activities.
  • Identifying individualised needs and outcome planning.
  • Knowledge of the social care sector.
  • Knowledge of the voluntary sector.
  • Interpersonal skills and the ability to build and develop relationships.
  • A collaborative approach to work.
  • Honest, respectful, reliable, non‑judgmental.
  • Awareness of professional boundaries.
  • Flexibility with working hours.
  • Good working knowledge of Microsoft Office & CRM systems.
  • Ability to be creative when working with residents.
  • A full UK driving licence is required for this role.

  • Ability to deal with challenging behaviour with a non‑confrontational approach.
  • Awareness of the need for outcomes.
