The Move-On Coordinator is a specialist role within the Osborne Grove Assessment Centre, working as part of a wider multi-disciplinary team that includes the Service Manager, Deputy Manager, Case Coordinator, Assessment Workers and Move-On Workers. The post holder is exclusively responsible for coordinating move-on activity once assessments have been completed and individuals are deemed ready to progress. The role does not undertake initial assessments; instead, it provides dedicated oversight, coordination and leadership of the move-on process to ensure timely, safe and sustainable transitions into appropriate accommodation.

The Move-On Coordinator acts as the central point of contact between internal teams and external partners, ensuring that all elements of the move-on pathway are aligned, communicated effectively and progressed without delay.

Main Responsibilities

  • Take lead responsibility for coordinating all move-on activity for residents of the Osborne Grove Assessment Centre once assessment and care planning processes are complete.
  • Work closely with the Service Manager, Deputy Manager and Case Coordinator to ensure move-on plans are clear, agreed and progressed in line with service priorities and targets.
  • Act as the primary liaison with Supported Housing Pathway Managers to organise, progress and unblock move-on placements across statutory and non-statutory accommodation pathways.
  • Coordinate communication and joint working with Adult Social Care to ensure individuals with eligible care and support needs can access appropriate provision under the Care Act 2014.
  • Develop and maintain strong working relationships with private rented sector (PRS) agencies and landlords, to secure suitable accommodation and financial support where applicable.
  • Lead on organising and coordinating move-on arrangements, including multi-agency planning, confirmation of placements, and practical arrangements to enable safe and timely transitions.
  • Facilitate and contribute to multi-agency meetings involving statutory, voluntary and community sector partners to resolve barriers and progress complex move-on cases.
  • Monitor and track move-on activity, ensuring progress is recorded accurately and outcomes are reported to the Rough Sleeping Programme Lead.
  • Maintain high-quality case records and ensure all move-on activity is compliant with data protection, safeguarding and organisational requirements.

Knowledge, Qualifications, Skills, and Experience

  • Strong working knowledge of the Homelessness Reduction Act 2017 and the Care Act 2014. (Essential)
  • Experience of coordinating move-on from assessment centres, temporary or supported accommodation into longer‑term housing. (Essential)
  • Proven ability to work effectively with Adult Social Care, housing pathway managers and private rented sector partners. (Essential)
  • Experience of working with single homeless adults and people affected by rough sleeping. (Essential)
  • Excellent communication and coordination skills, with the ability to manage complex, multi‑agency processes. (Essential)
  • Strong organisational skills and attention to detail, particularly in tracking progress and outcomes. (Essential)
  • Experience of working with individuals with No Recourse to Public Funds or immigration‑related restrictions. (Desirable)
  • Knowledge of welfare benefits, housing‑related financial support and PRS incentive schemes. (Desirable)
