Homeless Link in Liverpool is seeking a SHAP Support Worker to deliver one-to-one support across residents' homes and community settings. You will manage a caseload, conduct welfare checks, risk assessments and support planning, with driving licence and access to vehicle required for travel between properties.
You will work with young people aged 18–25 who are experiencing, or at risk of homelessness, helping them move to independent living and access services.
