Join Our Caring Team at Homecarers Care Services. Are you passionate about helping others and making a real difference in people's lives? Homecarers Care Services is expanding its domiciliary care services across Manchester and Tameside, and we are looking for a dedicated and compassionate Part-Time Care Assessor/Mentor to join our growing team.
As a Care Assessor/Mentor, you will be responsible for assessing referred service users and taking a strength-based, person-centred approach to care planning and risk assessments. Your role will help enable individuals to live as independently as possible in the comfort of their own homes.
About the Role
- Assessing customers' needs and developing personalised support plans, including risk assessments, using a strength-based approach.
- Creating flexible care plans that allow customers to make day-to-day changes to their support arrangements.
- Using a range of information sources to identify whether the care being provided continues to meet customers' needs.
- Providing practical, hands-on assistance when required to complete tasks.
- Supporting service delivery as part of contingency planning arrangements.
- Keeping up to date with legislation, regulations, and best practice within adult social care, including CQC requirements and contractual changes.
- Maintaining effective working relationships with colleagues across the organisation, including Compliance teams, to ensure excellent communication and service delivery.
What We’re Looking For
- Previous care experience and a minimum Level 3 Diploma in Health and Social Care.
- Experience of assessment and care planning based on the principles of a strength-based approach.
- Experience working with health and social care professionals, including District Nursing teams and Allied Health Professionals.
- Flexibility to work a range of shifts, including alternate 1 in 3 weekends.
- Strong communication and interpersonal skills.
- A broad understanding of compliance requirements within the social care sector.
- A valid UK driving licence and access to your own vehicle.
- A genuine passion for delivering high-quality care.
