As a Care Coordinator, you will play a critical role in ensuring that service users receive high-quality, person-centred care. Working closely with the Registered Manager and Deputy Managers, you will coordinate and manage care schedules for service users considering their individual needs, preferences, and goals. You will be responsible for organising rotas, shift patterns, liaising with health professionals, supporting home visits, and assisting with the creation of care plans.
Daily duties will include:
- Coordinating care schedules and staff rotas
- Matching carers to our service users based on needs and preferences
- Maintaining accurate records and compliance requirements
- Liaising with service users, families, care staff, and healthcare professionals
- Responding to changes in care requirements and operational needs
- Ensuring high standards of care and customer service are maintained
What We’re Looking For
- Previous experience in Care Coordination in a Domiciliary care provider
- Excellent communication and organisational skills
- Ability to work effectively under pressure and manage competing priorities
- Good IT skills and experience using rota or care management systems
- A positive, flexible, and team-focused approach
- Knowledge of the care sector and CQC requirements
- It is desirable you have experience using People Planner/Access care

- Shift patterns: 09:00 – 17:00 Four days a week with one 14:00 – 22:00 with alternative weekends
