Support Worker
Job Purpose
To provide person-centred care and support to individuals, helping them maintain their dignity, independence, wellbeing and quality of life while ensuring their safety and comfort.
Key Responsibilities
- Provide compassionate, person-centred support based on each individual's care plan.
- Assist with daily living activities and encourage independence wherever possible.
- Support with personal care, including washing, dressing, toileting and continence care, where required.
- Assist with mobility, transfers and use of appropriate moving and handling equipment.
- Support individuals with meals, drinks and maintaining good nutrition and hydration.
- Provide companionship and emotional support.
- Support individuals to attend appointments, activities and access the community where required.
- Encourage participation in hobbies, social activities and meaningful activities.
- Support individuals with dementia, learning disabilities, physical disabilities, mental health needs and other support requirements.
- Observe and report changes in physical health, behaviour, mood or wellbeing.
- Recognise and report safeguarding concerns, falls, pressure damage, accidents or other risks.
- Follow care plans, risk assessments, medication procedures and safeguarding policies.
- Maintain accurate and up-to-date care records and daily notes.
- Maintain confidentiality and respect the individual's privacy and dignity.
- Promote equality, choice, independence and person-centred care at all times.
- Work effectively with colleagues, families and other professionals involved in the individual's care.
- Maintain a safe, clean and comfortable environment.
- Attend training and maintain required knowledge and skills.
Person Specification
- Previous UK care or support work experience preferred.
- Caring, patient, reliable and compassionate.
- Good communication and listening skills.
- Able to work independently and as part of a team.
- Respectful and non-judgemental attitude.
- Flexible and willing to work different shifts.
- Right to Work in the UK.
- Relevant care qualifications or training are desirable. xsabvtc
