Job Description Key Responsibilities Care Coordination Support Assist in conducting care needs and risk assessments for new and existing Service Users. Help develop, implement, and review personalised care and support plans that reflect Service Users goals and preferences. Ensure plans are outcome-focused and tailored to individual needs, promoting independence and wellbeing. Scheduling and Service Delivery Support the creation and maintenance of staff rotas to ensure efficient service delivery. Work with the team to allocate suitable carers based on skills, continuity, and Service User compatibility. Respond promptly to changes in care needs or staff availability, updating schedules as needed and be willing to step in and cover when needed. Communication and Liaison Serve as a point of contact for carers, Service Users, and families, addressing routine queries and concerns.
