Support Worker Job Purpose

To provide person-centred care and support to individuals, helping them maintain their dignity, independence, wellbeing and quality of life while ensuring their safety and comfort.

Key Responsibilities

Provide compassionate, person-centred support based on each individual's care plan.

Assist with daily living activities and encourage independence wherever possible.

Support with personal care, including washing, dressing, toileting and continence care, where required.

Assist with mobility, transfers and use of appropriate moving and handling equipment.

Support individuals with meals, drinks and maintaining good nutrition and hydration.

Provide companionship and emotional support.

Support individuals to attend appointments, activities and access the community where required.

Encourage participation in hobbies, social activities and meaningful activities.

Support individuals with dementia, learning disabilities, physical disabilities, mental health needs and other support requirements.

Observe and report changes in physical health, behaviour, mood or wellbeing.

Recognise and report safeguarding concerns, falls, pressure damage, accidents or other risks.

Follow care plans, risk assessments, medication procedures and safeguarding policies.

Maintain accurate and up-to-date care records and daily notes.

Maintain confidentiality and respect the individual's privacy and dignity.

Promote equality, choice, independence and person-centred care at all times.

Work effectively with colleagues, families and other professionals involved in the individual's care.

Maintain a safe, clean and comfortable environment.

Attend training and maintain required knowledge and skills.

Person Specification

Previous UK care or support work experience preferred.

Caring, patient, reliable and compassionate.

Good communication and listening skills.

Able to work independently and as part of a team.

Respectful and non-judgemental attitude.

Flexible and willing to work different shifts.

Right to Work in the UK.

Relevant care qualifications or training are desirable.

DBS check required.

Working Hours

Flexible shifts may include days, evenings, nights, weekends and bank holidays , depending on the client's requirements.
