HomeCareDirect are currently seeking a Regional Care Nurse Lead to join their team on a full‑time basis. You will be home and community based and will work autonomously, managing your own caseload of clients living with a range of complex health needs in the Brighton and Hove area. Your role is to ensure that the people you support are given the best opportunities to remain healthy and out of hospital, whilst assisting them to ensure their lives are enriched and they are achieving their goals. This is a support management position to help people to live an independent life in line with their care and support plan.
Your role is to ensure that the people who use the HomeCareDirect service are supported to achieve their goals and aspirations to care standards whilst making a success out of directly their own support at home. You will have a client caseload to provide ongoing visits to create care and support plans, undertake annual support reviews, risk assessments, personal assistant inductions, supervisions, team and MDT meetings and other visits as necessary to keep the person safe.
Key Tasks- Support the person (and their family where appropriate) to live independently and direct their own support whilst working closely with the HomeCareDirect Independent Living Team to solve problems with a pro‑active 'can‑do' attitude and approach.
- Support the person in their support at home, to meet their assessed needs.
- Be pro‑active to improve the quality of life of the person living at home.
- Visit prospective new service users at home or in hospital to provide information and advice on how HomeCareDirect can help people to live independently at home.
- Create the care and support plan to meet the assessed needs in partnership with the person whilst assessing the training needs of their personal assistant(s) to the professional standards to which you are qualified.
- Undertake risk assessments for both the person and their personal assistant(s) and report any non‑compliance to head office.
- Complete documentation to the timescales required to meet professional standards.
- Work in partnership with the HomeCareDirect head office to help ensure that Health & Safety standards are met.
- Undertake interviews and supervision of personal assistant(s) in partnership with the client.
- Undertake clinical observations of the personal assistant(s) in line with their role.
- Visit the client as often as indicated in the support plan with a minimum of twice yearly supervisions for personal assistants.
- Undertake annual support reviews to CQC standards.
Essential Qualifications/Experience/Personal Qualities:
- You will be a qualified nurse with a minimum Band 5 registration.
- Registered with the NMC with a valid PIN.
- Have clinical, complex care experience.
- Have a commitment to undertake continuing professional development (CPD).
- You will need to have extensive experience of working in the community as a lone worker and in a senior role.
- Have good IT skills, fully conversant with Microsoft Office software, with internet access to enable use of our bespoke online care planning system.
- Have some care‑planning experience.
- Able to provide personal assistant training in line with your competencies.
- Have a full, UK driving licence.
- Have a compassionate and caring outlook with the ability to relate well to people, with families and personal assistants, in a supporting role.
- Be a good listener to the views, opinions and needs of the individual (or family where appropriate).
- Be flexible and non‑judgmental in accepting individual situations and environments.
- Good attention to detail, with a high personal standard for service quality whilst ensuring you follow the policies and procedures of HomeCareDirect.
- Able to talk to people at a professional level.
- Be flexible and willing to make necessary changes to meet the changing needs of the company.
- Must live in the Brighton and Hove region or be willing to relocate.
A salary of: £44,850, increasing to £48,750 per annum after a successful 6 month probationary period. Mileage is paid at 48p per mile for the first 10,000 miles, 25p per mile thereafter. Food allowance for overnight stays are paid at £8.99 for breakfast and £20.00 for evening meals. Overnight stays where required, are paid for by HomeCareDirect via our corporate account. Contribution towards professional registration of £80.00 per annum. Holiday entitlement of 20 days plus 8 bank holidays which increases to 25 days plus 8 bank holidays after 1 year of service.
