What You’ll Do

  • Play a critical role in supporting new customers integration, ensuring excellent standards of care from day one of a customer’s journey with HSM.
  • Providing short‑term care and support, focusing on new customer integration.
  • Building strong rapport with customers and their families from the outset.
  • Identifying risks, safeguarding concerns and any required interventions.
  • Communicating clearly with branch teams, field care supervisors and other professionals.
  • Ensuring accurate, timely documentation that meets regulatory standards.
  • Representing Home Support Matters with professionalism, empathy and expertise.
