Home Instead is seeking a dedicated Health Care Provider to support care consultations, service reviews and the creation, update and audit of client care plans. You will supervise staff, maintain accurate records in Home Instead software and People Planner, conduct client introductions, reviews and mentor Care Professionals.
The role requires Level 3 NVQ in Health and Social Care or equivalent, excellent communication, IT proficiency (Microsoft Office / Google Suite) and a full driving license
