We're looking for a Care Coordinator to join our ever-growing team. Are you organised, caring and ready for your next step? Become part of our coordinating team, keep our rota running smoothly and help shape the care our clients receive every day. Come and join an outstanding team!
Your main job: scheduling
As part of our coordinating team, most of your time will be spent on the rota: covering the day-to-day rota and working ahead to plan future rotas, so every client gets the right care at the right time.
- Covering the daily rota, including sickness, changes and last-minute calls
- Working ahead on rotas, so clients and Care Professionals know who is coming and when
- Protecting continuity, so clients see familiar faces and Care Professionals keep their regular rounds
- Speaking to clients and their families about any changes before they happen, and recording every change
- Making sure calls match each client's care plan, with realistic travel time and fair, safe shifts
- Supporting with care calls when needed to cover sickness and annual leave
- Being part of the on-call rota, around 1 week in every 5-6 weeks
Also part of the role: compliance and quality
- Checking Care Professionals' compliance, including training, supervisions and spot checks
- Completing supervisions with Care Professionals
- Going out to support with compliance, such as support visits in clients' homes
- Completing service reviews with clients and their families
- Carrying out quality assurance checks over the phone
Supporting our team
- Checking in on our Care Professionals' wellbeing, and being a friendly, supportive point of contact
- Working as part of a close, caring coordinating and office team
What we're looking for
- Experience in care. Coordinating or scheduling experience is a bonus, but full training will be given.
- Great organisation and the ability to stay calm when things change quickly
- Excellent communication skills on the phone and in person
- A caring, person-centred approach, with high standards for safe, quality care
- Confidence using computer systems (we use Access Care Planning)
- A full driving licence and access to a car (essential)
- Happy to support with care calls and to join the on-call rota
What we offer
- 37.5 hours a week
- Pay: Β£28,000 - Β£30,000 per annum with additional pay when On Call
- Full training and support from an experienced team
- The chance to develop your career in coordination, compliance and care management
- A supportive company that puts its clients and its team at the heart of everything it does
