Role Description

This is a full-time, on-site role for a Care Coordinator located in Pocklington. The Care Coordinator will be responsible for managing the schedules and coordination of care for clients, ensuring their needs are met with efficiency and compassion. The daily tasks include care planning, case management, direct communication with clients and their families, and collaboration with the caregiving team. The role also involves providing excellent customer service and ensuring home care services are delivered to the highest standards.

Qualifications

• Proficiency in Care Coordination and Case Management to plan, schedule, and oversee client care services
• Strong Communication skills to clearly interact with clients, families, and team members
• Knowledge of Home Care practices and experience in providing quality care to older adults
• Customer Service skills to address client inquiries and deliver exceptional support
• Ability to work effectively in a fast-paced, on-site environment
• Previous experience in a similar role, using Access and familiarity with the aging population\’s needs are a plus
• Proficiency in organizational and problem-solving skills
