Job Description – Support Coordinator (25880) Position: Lead Support Worker Location: Surrey – Guildford Type: Permanent, Part Time (17 hours per week), Rota Working (Monday to Sunday, 7am – 7pm) Salary: £13.21 per hour (equivalent to £25,838 per annum pro-rata) plus benefits including Health Cash Plan About Us: Home, a place where you belong Are you passionate about supporting individuals to improve their lives? As a Support Coordinator, you will support customers with complex needs to develop skills for independent living, making a positive impact daily. Responsibilities: Meet with customers to create and review support plans based on their needs. Assist with daily activities such as meal prep, shopping, budgeting, and maintaining tenancies. Conduct risk assessments, support planning, and goal setting. Engage in meaningful conversations and collaborate with colleagues. Oversee property maintenance and safety checks. Requirements: Passion for supporting customers to achieve their best. Experience in creating person-centered support plans. Ability to coordinate referrals and work independently. Additional Details: Flexible working arrangements; occasional adjustments to start/end times may be needed. Work on a rota basis, including Bank Holidays and Christmas.
