The service provides person centred support to those with complex mental health needs to develop their skills to live more independently in the community. You’ll make a difference every day to our customers' lives, enabling them to achieve their aspirations.
Typical day as our Support Coordinator:
- Creating person centred support plans with our customers, working with our clinicians to develop customers Risk Management Plans.
- Supporting customers with independent living skills such as budgeting, maximising customers' income, cooking, engaging with external services.
- Promoting Home Group’s values within your team.
- Working in close collaboration with local authority, NHS mental health teams, safeguarding teams and housing option teams.
You bring:

- Experience of coordinating and signposting to internal and external services.
- The ability to work on your own initiative, remain calm under pressure and have a resilient approach.
- Recognise every customer is different and you’re able to adopt a flexible, personal approach, listening to customers to understand their needs.

Job details:
- Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
- Working to a set rota pattern of day shifts, including weekends and Bank Holidays.
- Able to use technology to update outcome spreadsheets, complete online learning and to collaborate with colleagues and our stakeholders within the city.
- You’ll need an Enhanced DBS check done and we pay for that.
