- Create support plans with your customers within service or out in the community.
- Signposting and supporting customers to our internal and external partners.

You have:
- Passion for and experience in supporting others within supported housing settings.
- Passion to support our customers to live their best life, working collaboratively with an eye for detail.
- Experience of creating person-centred support plans and supporting customers in working to their individual plans.

- The ability to work on your own initiative, remain calm under pressure and have a resilient approach.
Stronger together:

The practical bits:
- The role is primarily based over 5 days - 7.5hrs/day (9.00am - 5.00pm) but we are open to flexible working hours.
- Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
- Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
- You’ll need an Enhanced DBS check done and we pay for that.
- To get from A to B, you’ll need a vehicle insured for business purposes. The great news is that we’ll pay your mileage! If you don’t drive that’s not an issue, we will cover your other travel expenses as well.
