What you\’ll do

  • Create person-centred support plans and lead a small team to deliver them

  • Support customers with mental health, substance use, homelessness and isolation

  • Help people build daily living skills and confidence in their community

  • Carry out risk assessments, reviews and safeguarding checks

  • Set up and end tenancies, making sure customers feel safe and supported

Practical bits

  • You\’ll work Monday to Sunday on a rota-based system

  • You\’ll work 37.5 hours per week on a rota-based system

  • You\’ll cover a patch across services in West Yorkshire

  • You need a vehicle insured for business use, and we pay your mileage

  • You need an Enhanced with barring list DBS check that we pay for
